This contract has been designed for groups of 25 or more individuals, in which the member and his or her eligible dependents agree to coverage for medical services received according to the plan.

Diverse Treatments:

• Alcohol or drug detoxification, except at hospitals that exclusively treat substance dependence
• AIDS treatment, up to a maximum of 15 percent of covered benefits

Pediatric care:

• Congenital diseases for eligible children under contract terms
