Description

Prepaid medical services which include US$ 2,000 life insurance for the primary member.

Eligibility

• Any person younger than age 65.
• May include spouse or live-in partner younger than age 65.
• Dependent children, from the day 10 after birth up to age 25, single and full-time students.

Benefits

• Automatic benefit as primary member or children as dependents, younger than age 25 or married.
• Reinstatement at 25 percent of Maximum per Disability, presenting proof of health that meets company’s requirements.
• If the primary member passes away, the eligible dependent family members will have at their disposition –free of cost- the plan for 12 months starting on the date of the member’s death.
• Ophthalmologic benefit limited for the primary member with a maximum annual benefit of US$ 100, maximum of three per family (US$ 300 per year per family).
• Benefit for dependent children studying abroad is included. Only applies to Plan I.
• Preventive Examinations:
  • Up to six examinations per year for children up to age 6.
  • Mandatory vaccinations: tuberculosis, tetanus, poliomyelitis, hepatitis, among others.
  • Annual examination with gynecologist. Papanicolaou smears, maximum of two per year and mammography test starting at age 40.
  • Annual examination with urologist. Prostate specific antigen test starting at age 40.

Telephone Medical Assistance

• Mediphone 24 – Among various services, it offers:

  • Guidance service and telephone medical advice 24 hours a day, 365 days per year.
  • Home health medical support in cases of emergency and first aid (maximum of two deliveries per year).
  • Patient referral to medical specialists depending on pathology.
  • Advanced life support units in case of emergencies.
  • Delivery coordination of: at home medications, among others.

• Travel Assistance– Among other services, it offers:

  Travels outside Ecuador that last no longer than 90 days. (Please refer to the details and limitations of these benefits as specified in the plan)

  • Repatriation in case of injuries, illness or death.
  • Medical assistance for injuries or illness contracted in foreign countries.
  • Medical ground transportation.
  • Hotel expenses due to convalescence.
  • Legal bails advance payment.
  • Tracking down and transportation of lost or stolen luggage.
  • Additional compensation for loss of luggage.
